Form 1095-C Information
Form 1095-C is a statement of health coverage offered to eligible employees. The ACA requirement to distribute Form 1095-C is no longer mandatory, nor is the form required for an individual to file a tax return. The IRS distribution rule has been changed to only being required "upon request" of the covered individual or full-time employee.
